form DL-180C; signed and notarized.

If you are using one of our Four Star Driving School vehicles for your exam, registration & insurance documents will NOT be needed.

EXAM CANCELLATION POLICY

$70

Any exam that is cancelled and/or rescheduled by the applicant (or applicant's parent/guardian) less than 48 hours before the scheduled exam time will be subject to a seventy dollar ($70) late cancellation fee.

NO SHOW POLICY

$150 / $200

Any applicant, who does not show up for their scheduled exam, will be subject to a one hundred and fifty dollar ($150) late cancellation fee.
Any applicant, who does not show up for their scheduled exam with prep, will be subject to a two hundred dollars ($200) late cancellation fee.
